It is extremely important that we have accurate and up-to-date information in our student database in the event we would need to reach you in an emergency. In place of the paper forms, we provide an online tool that will allow you to update your emergency contact information. This will better protect the security of your personal information, and expedite processing of any changes you make.

Please log in to your Parent Portal account by visiting your school website or the District website at: http://lewispalmer.org and click on the MyCampus link at the top of the page. There, you will find a link titled, Emergency Contact Verification Form, where you may review and make changes to your emergency contact information. Please update all telephone numbers, emergency contact names, etc. using this tool. We will need you to also check a box for each child confirming that you have reviewed their information online. If you do not have a MyCampus Parent Portal password, you may sign up for an account online under the MyCampus menu tab.

 If you would prefer to update this information by reviewing a paper copy of this information, you may obtain a copy of the form from your school office or district administration building.
